Making it Personal Practice development session – Autumn 2016

These two sessions bring health and social care practitioners together in a learning environment to explore why personal outcomes and asset based approaches are a central feature in health and social care.

Irrespective of professional discipline, there exists a core set of values, principles and skills which enable practitioners to focus on what matters most to people and what they want to achieve from support. The sessions explore how to harness the role of the person: their strengths; social networks; and community supports.

These skills enhance practitioners’ abilities in assessment and planning of support, rehabilitation as well as supporting a focus on wellbeing, recovery, prevention, anticipatory care and support for self management.

Who are the sessions aimed at?

These sessions are open to frontline practitioners, team leaders and managers from all sectors, health and social care, voluntary and independent.
