Supporting supported self management
We regularly work to help people develop practice that better supports people to learn how to self manage with a long term conditions.
This work develops practice by:
- Introducing practitioners to emerging ideas relating to person centred supported self management
- Stimulating reflection and conversation between practitioners
- Offering a supportive environment to exchange challenges and share best practice
- Providing information and resources
- Promoting positive changes in practice

- Be a facilitator - adopt a facilitative, motivational approach over an authoritative, didactic one
- Connect with local networks - encourage people to make use of community services and self management groups
- Support risk management - recognise risk taking as part of normal life and recovery
